Le balado est né de deux choses : des suicides de médecins autour de lui, et son propre épuisement. Des médecins, des infirmières, des résidentes, des psychologues et des proches y racontent la fatigue, le silence, la dépendance, la plainte, le retour. Des entrevues longues, d’une demi-heure à près d’une heure et demie, où l’on prend le temps. Vingt-deux épisodes et deux épisodes spéciaux à ce jour.

Trente-sept mille téléchargements audio et soixante-deux mille visionnages sur YouTube depuis le lancement, en date de septembre 2026.

Deux hommes échouent sur une île après un naufrage. L’un fait des cartes, l’autre coupe du bois. Deux mille mots, quatorze chapitres et un épilogue, qui se lisent en vingt minutes et se relisent longtemps. Après le témoignage et le balado, un conte, parce que certaines choses se disent mieux sans être nommées : deux hommes qui font avec ce qu’ils ont. Écrit et fabriqué d’abord pour un seul lecteur.

On raconte qu’il existe, au-delà des routes connues et inconnues, un trésor. Personne ne sait à quoi il ressemble. On raconte qu’il est inestimable. Qu’il est irremplaçable. Qu’il est introuvable.

L’édition numérique ajoute au conte une section de questions en trois niveaux, sans réponses, pour la table, la classe ou un soir seul, et un cahier de fabrication. Les illustrations ont été conçues et dirigées par l’auteur et réalisées avec l’aide de l’intelligence artificielle; le livre le dit et montre les images écartées.

Sur invitation

Devant des établissements de santé, des facultés, des congrès, des associations ou des groupes de résidents, Steven Palanchuck parle de la détresse de celles et ceux qui soignent, de la culture du silence en médecine et du retour après l’arrêt, à partir de ce que le balado lui a appris et de ce qu’il a vécu. Conférence, atelier ou discussion avec la salle, selon le lieu et le temps; la rencontre peut aussi prendre la forme d’un épisode du balado enregistré devant public, avec un invité, diffusé ensuite.

Troubles fonctionnels

L’autre moitié du travail, celle qui se fait en consultation

Une part croissante de sa clinique externe de médecine interne est consacrée aux troubles fonctionnels et aux syndromes persistants multisystémiques : syndromes post-COVID et post-infectieux, douleur nociplastique, dysautonomie et POTS, syndromes post-traumatiques, terrains d’hypermobilité.

Ces patients arrivent avec un dossier épais et la même phrase : on m’a dit que tout était normal. Le fil est le même que celui du balado, à une autre place : ce qui arrive à quelqu’un qu’on ne croit pas.

Les consultations se font sur référence médicale, par les voies habituelles du réseau. Ce site ne permet aucune demande directe et ne répond à aucune question clinique individuelle.

Steven Palanchuck est médecin spécialiste en médecine interne générale à Santé Québec Montérégie-Centre et chef du service des soins intensifs de l’Hôpital du Haut-Richelieu, où il a aussi fondé le service de médecine interne générale. Il est professeur adjoint d’enseignement clinique à l’Université de Sherbrooke. En novembre 2024, il a raconté dans La Presse ce qu’il avait traversé. Il a créé ensuite le balado documentaire Soigner jusqu’à se briser, tient un blogue dans Profession Santé, et a écrit L’Île, son premier livre. Il vit à Montréal et travaille en Montérégie.

The podcast was born of two things: physician suicides close to him, and his own burnout. Physicians, nurses, residents, psychologists and family members talk about exhaustion, silence, addiction, complaints, and coming back. Long conversations, half an hour to almost an hour and a half, where there is time. Twenty-two episodes and two specials so far.

Thirty-seven thousand audio downloads and sixty-two thousand views on YouTube since launch, as of September 2026.

Two men are washed up on an island after a shipwreck. One draws maps, the other cuts wood. Two thousand words, fourteen chapters and an epilogue, a twenty-minute read that rewards rereading. After the testimony and the podcast, a tale, because some things are better said without being named: two men making do with what they have. Written and made for one reader first. Translated from the French by the author.

They say there is, beyond the known and unknown routes, a treasure. Nobody knows what it looks like. They say it is priceless. That it is irreplaceable. That it cannot be found.

The digital edition adds to the tale a set of questions in three levels, with no answers, for the dinner table, the classroom or an evening alone, and the notes on how the book was made. The illustrations were conceived and directed by the author and made with the help of artificial intelligence; the book says so and shows the pictures that were set aside.

By invitation

Steven Palanchuck speaks to health care institutions, medical faculties, conferences, professional associations and resident groups about the distress of those who care for others, the culture of silence in medicine, and coming back after a leave, drawing on what the podcast has taught him and on what he has lived. A talk, a workshop or a conversation with the room, depending on the place and the time; the meeting can also take the form of a podcast episode recorded before a live audience, with a guest, and released afterwards. In French; in English on request.

Functional disorders

The other half of the work, the part that happens in clinic

A growing share of his outpatient internal medicine practice is devoted to functional disorders and persistent multisystem syndromes: post-COVID and post-infectious syndromes, nociplastic pain, dysautonomia and POTS, post-traumatic syndromes, constitutional hypermobility.

These patients arrive with a thick file and the same sentence: I was told everything was normal. The thread is the same as the podcast’s, in a different seat: what happens to someone who is not believed.

The clinical framework, the patient guide and the resource directory are in French, since the services they point to are Quebec services. The resource finder flags services available in English. Consultations require a medical referral through the usual channels; this site takes no direct requests and answers no individual clinical questions.

Steven Palanchuck is a general internal medicine specialist at Santé Québec Montérégie-Centre and Chief of the Intensive Care Division at Hôpital du Haut-Richelieu, where he also founded the general internal medicine service. He is an adjunct clinical professor at the Université de Sherbrooke. In November 2024, he wrote in La Presse about what he had been through. He then created the documentary podcast Soigner jusqu’à se briser, writes a blog in Profession Santé, and wrote L’Île, his first book, which he translated as The Island. He lives in Montréal and works in the Montérégie region.
